Q: How do retrieve a message from my doctor? What is my ID number?
A: We use a system called "Medvoice" to help us communicate with patients. You will need an ID number to get your message. Your ID number is you social security number. If you do not give us your social security number, your ID will be the first 9 digits of your telephone number.
Q: Can I email my doctor?
A: We offer a optional service for a fee called Webview that allows secure web-based messaging between providers and patients. Communicating with your doctor is similar to email but is more secure. Additionally Webview gives patients access to the their medical record. For more information about Webview click here.
Q: How long has Kachina Family Practice been around?
A: Kachina Family Practice was started in 1996. It was previously located off of Ray & 44th street and moved to our current location in 2006.
Q. Do you take walk-in appointments?
A: No. Our providers work from schedules so please call for an appointment. Same day appointments are usually available.
Q: Is there a discount for paying cash?
Q: I don't have insurance. Can you take partial payments for fee-for-service patients?
A: All fees that are the patient's responsibility are due at the time of service.
Q: I have a high deductible on my insurance, how will I be billed for my appointment?
A: Patients with a high deductible ($1000 or higher) will be required to pay 50% of charges at time of visit.
Q. You don't take my insurance but can you bill my insurance anyway?
A. No. We only bill our contracted healthcare plans. We will provide you with a copy of your charges so you can submit your claim to your healthcare plan.
Q. Can I come in to have my blood pressure checked without an appointment?
A: No. Blood pressure assessment in our office requires an appointment with your doctor. We suggest as an alternative going to the fire department or a grocery store/pharmacy if an appointment is not needed.
Q: Are the doctors board certified?
Q: How long have the doctors been in practice? Where did they go to medical school? Do they have any special medical interests?

Q: What is the difference between a doctor and a PA?
A: The scope of a Physicians Assistant's work corresponds to the supervising physician's practice. As part of their responsibilities, Physicians Assistants perform physical exams, diagnose illnesses, develop and carry out treatment plans, order and interpret lab tests, suture lacerations, prescribe medications, provider patient educations and preventative health care counseling. Q: I was in a car accident, can you bill the car insurance?
A: We do not bill 3rd parties or take liens such as billing car insurance companies for motor vehicle accidents. We will bill the patient's primary insurance company only. All other fees are due at the time of service.
Q: Can I see my doctor for a work related injury?
A: You can been seen by your doctor for any condition but we do not complete Workman's Comp forms/paperwork. If you feel you have a work related injury and would like to process a WorkmanÕs Comp claim, we suggest you go to Urgent Care or find out from your employer what clinic will handle your Workman's Comp claims.
Q: What should I do if something should happen over the weekend?
A: If you have a life-threatening emergency, you should dial 911. If you have an urgent need and need to speak with a Provider, you should call our office number at 480-706-4100 and you will be directed to the appropriate number for our answering service. The answering service will contact the doctor on call who will return your call and address your concern.
Q: I want to see a specialist, do I need a referral?
A: It depends on your insurance plan. If your plan requires a written referral, please make an appointment with your provider to evaluate the problem so that a referral can be started if needed.
