FREQUENTLY ASKED QUESTIONS
Q. Do you do 3rd party billing?
A. Kachina Family Practice does not bill 3rd parties or take liens such
as billing car insurance companies for motor vehicle accidents. We will
bill the patient's primary insurance company only. All other fees are
due at the time of service.
Q. Do you handle work related injuries?
A. Kachina Family Practice does not file Workman's Comp claims and we
do not keep Workman's Comp claim forms in our office. If you feel you
have a work related injury, we would refer you to Urgent Care or you
might ask your employer to refer you to an appropriate facility that
will handle your Workman's Comp claims.
Q. Do you bill or take partial payment on
Fee-For-Service accounts?
A. All fees that are the patient's responsibility are due at the time
of service. Patients with a high deductible ($1000 or higher) will be
required to pay 50% of charges at time of visit.
Q. Do you take walk-ins?
A. Kachina Family Practice Providers work from schedules. Our patients
are encouraged to call and schedule appointments. Same day appointments
are usually available.

Q. Do the Providers at Kachina Family Practice have
special medical interests?

Q. Is there a difference between an M.D. and a D.O.?
A. M.D. is the abbreviation for Doctor of Medicine. D.O. is the
abbreviation for Doctor of Osteopathy. In current times, there is
essentially no difference in the training of M.D.s and D.O.s. D.O.s are
not chiropractors. Some D.O.s and some M.D.s do osteopathic
manipulation, but this service is not offered at Kachina
Family Practice.
Q. What should I do if something should happen over the
weekend?
A. If you have a life-threatening emergency, you should dial 911. If
you have an urgent need and need to speak with a Provider, you should call our office number at
480-704-4100 and you will be directed to the
appropriate number for our answering service. The answering service
will contact the doctor on call who will return your call and address
your concern.
Q. My healthcare plan is
not one that Kachina Family Practice is contracted with, will you bill
my insurance company anyway?
A. No, we only bill the healthcare plans that we are contracted
with.
We will provide you with a copy of your charges so you can submit your
claim to your healthcare plan.
